Setting Up a Sign:
The'res two things you have to make sure of in order to make a chest spawn items: the format and the positioning. These pictures are pretty much self-explanatory:

Positioning:
The setup on the left is your default setup. Notice how the sign is only two blocks below the chest.
The setup on the right is four blocks below the chest and since that is not the default setup, it requires you to specify how many blocks the sign is below the chest on the last line of the sign.
