Holograms now re-appear on death

- individualholograms.deathmessage (required to auto-create death holograms)
- individualholograms.deathmessage.ignore (to not create a death hologram for you)
Added option for mob-damage holograms

should work for mobs...
IH is now packaged with an additional example placeholder
Fixed a bug with if statements not being evaluated properly in a custom placeholder
