Added the ability to heal all players with /heal *

Players must sneak to heal themselves with a sponge
Bug fixes and code cleanup
New sponge permissions:
hp.sponge.place: required for all other sponge permissions
hp.sponge.heal: required for healing yourself with a sponge
hp.sponge.feed: required for feeding yourself with a sponge
hp.sponge.fire: required for setting yourself off fire with a sponge
Console commands should work, but get a message saying "You do not have access to this command!" Just ignore this.
