Players that die while actively going through a dungeon drop their inventory on the ground, however the player re-spawns with their full inventory as well effectively dropping a dupped copy on the ground where they died.
I knew this inventory stuff was going to be a huge pain when it put it in. I'll look into this problem.

I've posted a new version (0.8.6) that should hopefully fix the duping issue you are seeing that was due to a minor bug in my logic. Unfortunately, after wrestling with the inventory management stuff for 2 days I am at a dead end and have had to add a disclaimer for anyone using the inventory management commands:

I am going to leave this ticket open as a reminder to eventually fix this provided either the bukkit api is improved or I have a revelation on how to implement this properly.
